IF YOUR EMPLOYER IS PAYING YOUR 2005 FEES
Contact your employer to find out if they require you to submit your form and fee directly to the SRNA or if they will remit it on your behalf. If your employer remits your fee, please give your form to your employer, as the SRNA requires that the form and fee arrive together. If the confidentiality of information on the form is a concern, it may be placed in a sealed envelope with your name and RN# on the outside and submitted to your employer for payment and forwarding to our office.
If you are submitting the form and fee, when you receive your receipt in the mail, submit it to your employer for reimbursement.
