Renewal Registration - December 1, 2007 - November 30, 2008
1) Employer Remitted Fees
Contact your employer to find out if they require you to submit your form and fee directly to the SRNA or if they will remit it on your behalf. If your employer remits your fee, please give your form to your employer as the SRNA requires that the form and fee arrive together. If the confidentiality of information on the form is a concern, it may be placed in a sealed envelope with your name and RN# on the outside and submitted to your employer for payment and forwarding to our office.
If you are submitting the form and fee, when you receive your receipt in the mail, submit it to your employer for reimbursement.
2) Single Payment
Cheque, Money Order - in the amount of $483.00 may be made payable to the SRNA. Payment by post-dated cheque is acceptable, however, will not be considered as received by the SRNA until the date denoted on the cheque. If you have not renewed your licence by November 30, 2007, you will cease to be a member of the Association. If you require registration after June 1 or September 1 please refer to fee schedule for pro-rated fees (Section 17 on the Membership Renewal Inventory Form). NSF Cheques - A fee of $40.00 is charged for all NSF Cheques. Failure to clear arrears within a four-week period will result in voiding of your licence, employer notification and refusal to verify eligibility to practise nursing.
Payment by Credit Card - The Association accepts Visa and MasterCard for payment of membership fees for the single payment option only. Include your complete card number (16 numbers) and expiration date. Incomplete information will result in delays. Credit card payments are not accepted for the two installment payment option. Declined Credit Cards - Please note if your credit card is declined by your financial institution, you will not be considered Registered until the funds have been received.
3) Two Installment Payment Plan
A $20.00 service fee will be added to the annual fee ($460.00 + $23.00 GST + $20.00 admin. = $503.00). Your first cheque - must be in the amount of $261.50. You must also enclose a second cheque - dated April 30, 2008 in the amount of $241.50. Mail BOTH cheques with your completed membership renewal inventory form. Please note that credit card payments will NOT be accepted for the two installment payment plan.
4) Payroll Deduction
Nurses who are participating in a payroll deduction plan for payment of 2008 licence fees should complete their membership renewal inventory form and return it to their employer before the agreed upon date in October. Your employer will forward the fee and forms to the SRNA for nurses who agreed to the plan. Once processing is completed, your licence will be mailed directly to you. Please note that unless you submit your completed renewal form to your employer by the required deadline, your 2008 registration/licence cannot be processed. For general information about your payroll deduction plan, please contact your employer directly.
Practising Membership Fees are pro-rated and are illustrated on the back of the renewal form. There is a further reduction if you have dual registration. In other words, if you currently have a 2008 practising membership in another Canadian jurisdiction, which affiliates with the Canadian Nurses Association, you are eligible for a reduction on your SRNA fees (excluding College of Nurses of Ontario and Quebec's registering body). Please include a photocopy of the signed 2008 practising membership card of the other province in which you are registered with this renewal form. Further documentation, namely Verification of Current/Most Recent RN Registration, may also be required.
GST Rebate
The 5% GST is added to all SRNA fees. Nurses whose employers are GST registrants will be able to file for a rebate of the GST at income tax time. Application for this rebate is made along with your Income Tax return. Please quote your employer's GST registration number. The specific form to request from Revenue Canada is form #GST 370 "Employee and Partner Rebate." You have four years to claim this rebate.
